Senior Health Requirement

Please be aware, the Illinois State Board of Education requires all students entering the twelfth grade to present proof of having a received a Meningococcal Conjugate Vaccination (MCV4) on or after their sixteenth birthday. This requirement must be met prior to October 15th or they will be excluded from school. You can get this vaccine at your family doctor and most chain pharmacies.

For those families that may require financial assistance, please contact Aunt Martha’s, the county health department, or the VNA. Well-child immunization appointments are limited and fill quickly, so please make an appointment as soon as possible.

When you go to the appointment, please make sure to get written documentation of the shot, including the date of the vaccine, signature of the provider, and the provider’s address and phone number stamped on the form. Once completed, please make a copy for yourself, and submit a copy to the Gail McKinzie Health Office at Tierney_Christerson@ipsd.org.
